李佃贵从浊毒辨治急性呼吸道感染经验

摘要:

总结李佃贵教授从浊毒辨治急性呼吸道感染的临证经验。认为急性呼吸道感染以浊毒为核心病机,以化浊解毒为基本治则,临证常以金银花、藿香、大青叶等达表透浊解毒,大黄、厚朴、枳实、槟榔等通腑泻浊解毒,薏苡仁、芦根等渗湿利浊解毒。以临床常见的急性呼吸道感染疾病流行性感冒、社区获得性肺炎为例,阐释浊毒理论的具体应用。针对流行性感冒,治以达表透浊解毒为法,方用自拟银翘透浊解毒饮加减;针对社区获得性肺炎,感染期治以清热泻浊解毒,方用自拟藿香化浊解毒饮加减;恢复期治以益气养阴、化浊解毒,方用自拟益气养阴败瘟汤加减。

Abstract:

This paper summarized the clinical experience of Professor LI Diangui in differentiating and treating acute respiratory tract infections from perspective of turbid-toxin. It is believed that turbid and toxin is the key machanism of acute respiratory tract infections, and resolve turbid and detoxin is the basic therapeutic principles. In the clinical treatment, Jinyinhua(Lonicera japonica), Huoxiang(Ageratum conyzoides), Daqingye(Strobilanthes cusia) often used to expel turbid from exterior and resolve toxins; Dahuang(Magnolia champaca), Houpo(Magnolia officinalis), Zhishi(Citrus aurantium L.), Binglang(Areca catechu) are used to unblock the bowels, discharge turbid, and resolve toxins; Yiyiren(Coix lacryma-jobi), Lugen(Phragmites australis) are used to drain dampness and turbid, and resolve toxins. Taking the common acute respiratory infectious diseases in clinic, influenza and community-acquired pneumonia, as examples, this paper explained the specific application of the turbid-toxin theory. For influenza, the treatment is to expel turbid from exterior and resolve toxins, and the self-prescribed Yinqiao Touzhuo Jiedu Decoction (银翘透浊解毒饮) with modification were used; for community-acquired pneumonia, in the infectious stage, the treatment is to clear heat, drian turbid, and resolve toxins, self-prescribed Huoxiang Huazhuo Jiedu Decoction (藿香化浊解毒饮) with modification were used; in the recovering stage, the treatment is to benefit qi and nourish yin, and resolve turbid and toxin, self-prescribed Yiqi Yangyin Baiwen Decoction (益气养阴败瘟汤) with modification were used.
