功能性便秘中医药临床及基础研究现状

摘要:

对中医药治疗功能性便秘(FC)文献进行综述,探讨FC中医药研究现状。发现临床研究多涉及中医证候、疗效研究,目前FC证候研究局限并缺乏高质量循证医学证据。现有循证医学证据显示中药内服、外治、针刺、穴位贴敷、埋线、推拿等治疗方法均能不同程度改善FC临床症状,临床疗效显著,且中西医结合疗法在FC的治疗中可产生协同效应。FC机制研究聚焦两大方向:一是动物造模,构建不同分型及中医病证结合模型,既评价药效,又揭示FC病理生理与中医证候的生物学基础;二是多组学整合,运用系统生物学、网络药理学、代谢组学、宏基因组学等手段,系统阐明中药(复方、单味药、活性成分)通过调控肠神经、胃肠动力、水液代谢、肠屏障、炎症及菌群代谢产物网络发挥治疗作用的分子靶点与信号通路。通过对FC的中医药研究现状进行分析,提出未来FC中医药研究的方向。

Abstract:

This paper reviews the literature on the treatment of functional constipation(FC) with traditional Chinese medicine(TCM) to summarize the current status of TCM research in this field. Clinical studies mainly focus on TCM syndrome patterns and therapeutic efficacy. Although research on FC-related TCM syndromes remains limited and lacks high-quality evidence-based medical support, existing evidence indicates that various TCM interventions, including oral herbal medicine, external therapies, acupuncture, acupoint application, acupoint embedding, and tuina massage, can improve FC-related clinical symptoms to varying degrees, with notable therapeutic effects. Moreover, integrative therapies combining TCM and western medicine demonstrate synergistic benefits in FC management. Regarding mechanistic research, one major approach involves animal modeling, including models based on different FC subtypes and combined disease-syndrome models, which serve not only to evaluate therapeutic efficacy and pharmacological mechanisms but also as important tools for exploring the pathophysiology of FC and the biological basis of TCM syndromes. Another approach employs systems biology, network pharmacology, metabolomics, metagenomics, and multi-omics methods to elucidate the molecular targets and signaling pathways of TCM compound formulas, single herbs, and their active components. These studies focus on regulation of the enteric nervous system, improvement of gastrointestinal motility, modulation of fluid metabolism, enhancement of intestinal barrier function, reduction of inflammation, and regulation of gut microbiota and their metabolites, thereby clarifying the pharmacodynamic and pharmacological mechanisms underlying FC treatment. Based on an analysis of the current research landscape, this paper proposes future directions for TCM research on FC.
